Neurocognitive Baseline Testing for Athletics: Saturday, August 4

Beginning this 2012-13 school year, ImPACT neurocognitive baseline testing for concussion will be required for all SJS students participating in Athletics here at school (7th grade through varsity). If your child has not previously completed baseline testing for his or her sport, testing will have to be completed prior to participation on a 2012-13 Maverick team.

Students entering the 7th grade will be tested the first week of school during PE period. Although welcome, these students do not need to attend the August 4th testing date.

We will be offering baseline testing here at school on Saturday, August 4 at 12:30 PM in the Middle School computer labs.

In preparation for testing, please access and print the 2012 ImPACT Baseline Consent Form and the 2012 ImPACT Baseline Demographics Worksheet from the Athletics section of the Parent Files and Forms section of the SJS web site. Complete these forms with the appropriate parent signature and send them with your student on testing day. Students will not be able to test without these completed forms.

For students in need of testing who will be unable to make this testing date, please contact Methodist Concussion Center over the summer at 713-441-8277 to make an appointment for testing at their offices. Testing is $5 and takes approximately 30 minutes.

If your child has previously completed baseline testing for participation on a SJS team, additional testing is not needed.
