8th Grade Students ImPACT Baseline Testing

In preparation for your child’s participation in 9th grade athletics in the Fall, we will be conducting ImPact baseline testing for all current 8th grade students during the week of April 28-May 1, 2014. Testing will take place in the MS computer lab during regular MS PE/Athletics period. If your child will not be attending SJS next year, or are certain they will not be participating in any athletics all year, please send me an email stating such, I and will remove them from the testing roster.
All SJS students participating on any Maverick athletic team are required to have a baseline ImPact neurocognitive test for concussion on file prior to participation. This is a simple computer based test of memory, attention and reaction time. Testing is done in collaboration with MethodistConcussionCenter. Students will need new ImPact tests every 2 years (7th, 9th, 11th grades).
In preparation for testing, please access and print the 2014 ImPACT Baseline Consent form and the 2014 ImPACT Baseline Demographics worksheet from the Athletics section of the Parent Files and Forms section of the SJS site. Please complete these forms with the appropriate parent signature. Please begin to turn in completed forms this week by sending them with your child to school to be turned in to his/her PE teacher/Spring coach. You may also scan and email completed forms to Melissa Russell at mrussell@sjs.org. Students will not be able to test without these completed forms.
