6th Grade Students ImPACT Baseline Testing

In preparation for your child’s participation in 7th grade athletics in the Fall, we will be conducting ImPact baseline testing for all current 6th grade students during the week of May 13-17, 2013. Testing will take place in the MS computer lab during regular 6th grade PE (3rd) period.
All SJS students participating on any Maverick athletic team (beginning in 7th grade) are required to have a baseline ImPact neurocognitive test for concussion on file prior to participation. This is a simple computer based test of memory, attention and reaction time. Testing is done in collaboration with Methodist Concussion Center.
In preparation for testing, please access and print the 2013 ImPACT Baseline Consent form and the 2013 ImPACT Baseline Demographics worksheet from the Athletics section of the Parent Files and Forms section of the SJS site. Please complete these forms with the appropriate parent signature. You may turn in completed forms by sending them with your child to school to be turned in to his/her 3rd period PE teacher. You may also scan and email completed forms to Melissa Russell at mrussell@sjs.org. Students will not be able to test without these completed forms.
