Middle & Upper School
If you are a Class 6-12 parent and have an expense that has been okayed by the Committee Chair or Class Chair, send the MS/US Reimbursement Request form to that person. He/She will complete the form and submit it to your class treasurer.

Lower School
If you are a Class K-5 parent and have an expense that has been okayed by your Room Mother, send the LS Reimbursement Request Form to her. She will complete the form and submit it to your class treasurer.
